Form used in examination of a patient have been based on Finnish translation from a form previously used at the British School of Osteopathy. Osteopathic diagnosis consist of history taking, physical examination and differential diagnosis. These lead to a working diagnosis about patients problem. All the information concerning patients complaints should be registered. Case history is the most important patient document. It contains all necessery information about the patient concerning the treatment. For workable data transmission and safe treatment a precise case history is indispensable. The purpose of this final project was to improve the form used in taking a case history and physical examination to correspond better with students needs during clinical practical training. Revision of the form was based on becoming acquainted with forms used in three different osteopathic schools in Europe, literature and tryout at Kunto-Stadia. This project was carried out in cooperation with teachers at the Kunto-Stadia and fourth year osteopath students. At the result of this project a reborn prediagnostic inquiry form was developed. The form is for the degree programme in osteopathy to be used in Kunto-Stadia teaching clinic.
